In this course, you will be thinking about chronic disease and illness. As you learn, you will consider different strategies for helping address the growing crisis in our communities and healthcare system. In particular, you will work to develop a proposal for a community outreach strategy, education program, event, or support center may help prevent or manage these types of illnesses. The guide will help highlight key resources to help with building an effective proposal.
